CDS and in Home Program Requirements
In order to qualify for the benefits of this program, clients must:
- Be 18 years or older
- Have active Missouri Medicaid or be eligible
- Have a physical disability that impedes daily activities
- Meet Department of Health and Senior Services (DHSS) qualifications for approvals
- Want to continue to live independently and versus living in a nursing home
- Have the desire and ability to make decisions regarding health care and employees (CDS)

Adult Day Care Services
Our adult day care facilities are ideal alternatives for seniors and people with disabilities. While their caregiver is out, they can stay here and mingle with the community. Visiting here allows them to take a break from their attendant, get some much-needed social interaction, and gain a better structure to their daily activities. They can also enjoy the following services:
- Assistance With Eating, Medicine, Toileting, and/or Walking
- Counseling
- Educational Programs or Mental Stimulation
- Exercise Programs

Eligibility Requirements
